Birmingham have major concerns over their midfield duo Barry Ferguson and Craig Gardner ahead of the game.
Ferguson played the final hour of the Carling Cup win over Arsenal last weekend with a broken rib, while Gardner suffered a calf injury and was substituted early in the second half.
Alexander Hleb has also not recovered from the knee injury which kept him out of the final.
But winger David Bentley and defender Curtis Davies are available after being cup-tied against the Gunners.
West Brom midfielder Graham Dorrans is a major fitness doubt for the derby.
Dorrans suffered an ankle injury in training on Friday and Baggies head coach Roy Hodgson is pessimistic about his chances of being available.
Striker Peter Odemwingie suffered a knock to his ankle during the 1-1 draw at Stoke on Monday.
But he has returned to training after three days of treatment and should be fit to face Alex McLeish’s side.
